Population Health Management Project

Much Birch is taking part in an Integrated Care Board (ICB) Population Health Management project. This uses securely analysed health information to help the NHS better understand local health needs, improve services, and plan care more effectively. Information is drawn from GP practices, hospitals and social care, and is analysed in a way that does not identify individual patients. Only staff directly involved in your care at the Practice can see your identifiable information. You are automatically included, but you have the right to opt out at any time.
